PPH is generally caused by four major medical conditions, commonly referred as the "four Ts or 4 Ts" mnemonic: tone (uterine atony), trauma, tissue (retained placenta), and thrombin (coagulopathy). Uterine atony is the most common cause of PPH, accounting for almost 75% to 80% of primary PPH cases. Scientific data based on high quality evidence suggest that active management of the third stage of labor reduces the severity and incidence of PPH. Active management generally includes immediate administration of uterotonic agents (e.g. oxytocin, prostaglandins, and ergot alkaloids) post-delivery, early cutting and clamping of umbilical cord, and gentle traction to the umbilical cord.
