Reclast (zolendronic acid) is a bisphosphonate manufactured by Novartis for the treatment of osteoporosis in postmenopausal women and in men (Reclast Injection Package Insert, 2011). Reclast's defining attribute is its once-yearly administration, which puts it in a class of its own in terms of ease of use and patient compliance. The single dose, a 5mg/100mL injection, is typically administered intravenously for 15 minutes. Reclast works by rapidly binding to hydroxyapatite, a key structural component of bone, and by inhibiting farnesyl pyrophosphate (FPP) synthase. FPP synthase is inhibited by the prevention of post-translational modifications of small GTPases, which retards osteoclast function (Black et al., 2007).
